Medical Entity and Provider Network
OSI MEDICAL SERVICES, P.A & Scriptful Inc
The medical entity and provider network identified above provide access to independent licensed healthcare professionals who evaluate patients, determine medical appropriateness, provide clinical services, and, when appropriate, issue prescriptions based on their independent professional judgment.
OakMD is the technology and administrative platform operated by AB Brands LLC. OakMD does not direct, control, or interfere with the independent clinical judgment of healthcare professionals. All medical decisions, including whether a prescription or treatment is appropriate, are made solely by the treating licensed healthcare professional.
The applicable clinical practice, treating provider, and dispensing pharmacy are disclosed during the applicable care or fulfillment process as required by law. Prescription medication is dispensed by appropriately licensed pharmacy partners pursuant to a valid prescription; identifying the medical entity and provider network does not identify them as the dispensing pharmacy.

State-Specific Availability and Eligibility
Nationwide access to at least some services does not mean that every medical service, treatment, medication, dosage form, provider, pharmacy, shipping method, or subscription option is available in every state. Specific medical services, treatments, providers, and pharmacy partners vary by state and may not be available everywhere.
You must be physically located in a state where the treating provider is authorized to provide care at the time clinical services are rendered. The dispensing pharmacy must be authorized to dispense and ship the prescribed medication to that state. Availability is subject to applicable law, provider licensure, pharmacy authorization, patient eligibility, clinical appropriateness, and operational availability.
A provider may require a live video or telephone consultation, laboratory testing, medical records, an in-person examination, or other information before making a treatment decision. Completing an assessment, submitting payment information, or accessing the website does not guarantee treatment or a prescription.
